1. ASSUMPTION OF RISK

I, the undersigned participant and/or parent/guardian, acknowledge that participation in hockey training, strength and conditioning, skating treadmill use, shooting drills, and use of all equipment at Detroit Premier Hockey involves inherent risks, including but not limited to:

• Slips, falls, and collisions

• Injuries from equipment or pucks

• Muscle strains, sprains, or fractures

• Serious injury or death

I voluntarily assume all risks associated with participation in these activities.

2. RELEASE OF LIABILITY

In consideration of being allowed to participate, I hereby release, waive, and discharge Detroit Premier Hockey, its owners, employees, trainers, and affiliates from any and all liability, claims, demands, or causes of action arising out of any injury, damage, or loss, including death, that may occur while participating in activities at the facility.

This release applies to any claims resulting from negligence, to the fullest extent permitted by law.

3. MEDICAL FITNESS

I certify that I (or my child) am physically fit to participate in training activities and have not been advised otherwise by a qualified medical professional.

I understand that Detroit Premier Hockey does not provide medical evaluations or supervision.

4. USE OF EQUIPMENT

I agree to use all equipment, including but not limited to:

• Skating treadmill

• Weights and strength equipment

• Shooting lanes and synthetic ice

in a safe and responsible manner and follow all instructions provided by staff.

5. MINOR PARTICIPANTS

If the participant is under 18 years of age:

I, as the parent or legal guardian, consent to their participation and agree to all terms of this waiver on their behalf.

I accept full responsibility for any risks or injuries sustained by the minor.

6. INDEMNIFICATION

I agree to indemnify and hold harmless Detroit Premier Hockey from any claims, damages, or expenses arising from participation, including legal fees.

7. PHOTO & VIDEO RELEASE (OPTIONAL)

I grant permission for Detroit Premier Hockey to use photographs or videos of the participant for promotional purposes.

I have read this waiver carefully and understand its contents. I understand that I am giving up substantial rights, including the right to sue, and I sign it voluntarily.
